About the Role
Looking to join a lively and enthusiastic team that is focused on the future growth of Sunnyfield? As a Community Support Worker, you will have a crucial role in supporting and empowering individuals with disabilities to lead a life of their own choosing. Whether working as part of a team or independently, you will offer a variety of services that help clients live independently, actively participate in their local community, and engage in meaningful opportunities.
Key focus:
- Supporting clients to lead an active lifestyle and engage in their community through the implementation of various person-centered programs
- Implementing positive behavior strategies based on behavior support plans
- Assisting clients with personal care and hygiene
- Communicating with families regarding clients' progress
- Performing household and manual handling tasks like menu planning, cooking, and cleaning
- Maintaining progress notes and records
About You
