Clinical Care Coordinator

The Clinical Care Coordinator is responsible, in conjunction with the Home Manager to ensure the home meets clinical care needs in line with organisation and legislative requirements and has a key focus on quality assurance and continuous improvement.

Key Responsibilities:

  • Ensure that all care staff comply with their requirements and responsibilities to deliver best practice care,
  • Participation in the on-call roster, overseeing administrative tasks
  • To ensure that all areas of risk are communicated in a timely manner to the Home Manager,
  • To foster positive working relationships with all stakeholders and employees,
  • Ability to work independently and as part of a team
  • Overseeing and implementing quality improvement initiatives to ensure high quality of services.

Qualifications:

  • Current and valid AHPRA registration
  • Minimum of 1-2 years’ experience working in a similar role.
  • Ability to work independently
  • Professional communication and listening skills
  • Compassionate, patient and caring nature,
  • High level of organisational and time management skills
  • Able to work in a fast-paced environment
  • Sound computer skills
