Salary Guide: Depending on experience
Location: Brisbane North area and Moreton Bay Council region
Your normal day will involve:
- Deliver nursing services and visit clients homes engaging in Medication Management, Wound Care, Palliative Care, Dementia Care, Chronic Conditions.
- Engaging in client relationship management to ensure a high-quality client experience
- Collaborating with Support Workers on client care outcomes and service satisfaction
- Developing, implementing, and reviewing care plans to meet clients' health care needs and promote well-being
- Providing leadership and support to the Client Care team, including training and performance feedback
- Monitoring service levels, including funding and client/carer satisfaction
- Building and maintaining excellent relationships with clients, their families, funding bodies, and other stakeholders
- Promoting in-home support services and community programs, and developing new relationships
You will need:
- 4 years demonstrated experience in Community Services Coordination or Care Management
- Registered nurse or allied health professional
- Demonstrated experience supervising, supporting, and training staff
- Knowledge of WHS legislation relevant to Aged Care and Disability service sector
- Relevant qualifications in Community Services, Frontline Management, or equivalent
- AHPRA Registration, National Police Certificate, CPR certificate, and required vaccinations
- Current driver's licence and availability of a registered vehicle with comprehensive insurance
