Overview

It also acknowledges the cultural determinants and particular complexities affecting the health and wellbeing of Aboriginal people and the need for a holistic and multi-faceted health approach to service design and delivery.

SA Health is leading significant reform designed to improve the health of all South Australians and meet future challenges, in accordance with the National Aboriginal and Torres Strait Islander Health Plan and Close the Gap initiatives.

OPS5: The Senior Aboriginal Health Practitioner – Mental Health will work with multi-disciplinary team and leadership team within the RRMHS across the sites of BHFLHN and works closely with the Aboriginal Health Team leader/s to embed culturally relevant client focussed clinical care across the BHFLHN region ensuring the rights and needs of client are at the centre of service delivery across the multidisciplinary team including visiting specialists and referral pathways. The role will work with the Aboriginal Health Team and Aboriginal Project Team to implement BHF Aboriginal Health Strategy, Closing the Gap targets and priorities, and the Indigenous Australians Health Program KPIs. The role will support the embedding of these strategies and plans within a primary health care model by developing, consulting, implementing, and reporting on key performance measures and outcomes.

The Practice Manager has a strong focus on implementing and reviewing continuous quality improvement and clinical accreditation processes, including updating and maintaining practice manuals and creating and supporting resources for new clinical staff including Medical Students and GPex Registrars training within our LHN Clinics.

Working with the LHN’s Aboriginal Health Team Leader/s to oversee quality improvement projects across clinic operations ensuring broad team consultation, empowerment of staff, and client’s journey are at the centre of practice. The Practice Manager will support the development of new agreements, evaluation of existing agreements and negotiation of required changes with external parties as well as supporting strong connections with partnerships with external services.

OPS4:  The Aboriginal Health Practitioner (Practitioner) will work within the hospital setting to embed a stronger and more sustainable approach to support Aboriginal patients in hospital, or attending outpatients and emergency departments, through providing an ‘Aboriginal lens’ to health care and delivering cultural safe care. In collaboration with the multidisciplinary teams the Practitioner will assess and provide clinical intervention to Aboriginal patients, including those with complex needs. The service they provide will promote engagement and increase likelihood of Aboriginal client’s engagement with treatment and care plans. They will provide acute and sub-acute health responses, support hospital avoidance and advise on length of stay in the acute sector and closer to home services.

The Practitioner will provide clinical intervention, assessment, care planning and education within their scope of practice. The Practitioner reports to the Nurse Unit Manager and works collaboratively with the Rural and Remote Inpatient Unit Multidisciplinary team and the Barossa Hills Fleurieu Local Health Network Aboriginal Health Team. This role will contribute to South Australia’s Strategic Plan target 2.5 on Aboriginal life expectancy, as well as the Council of Australian Governments (COAG) National Partnership Agreement on Closing the Gap in Indigenous Health Outcomes health related targets.

The Practice Manager will provide day to day management, leadership and advice of health and wellbeing programs including education within their scope of practice and provide management support to a team of Aboriginal Health Practitioners. The Practice Manager reports to the Aboriginal Health Team Leader and works collaboratively with the Barossa Hills Fleurieu Local Health Network Aboriginal Health Team, Aboriginal Project Team, Medical Team, Allied Health Team and Aboriginal Family Birthing Team.
