Your Opportunity

The Psychosocial Support Program (PSP) is a vital program in the community in partnership with the Healthy North Coast PHN supporting people living with mental illness.

As the Clinical Care Coordinator, you will be the driving force behind our commitment to providing recovery-oriented support, ensuring every individual receives tailored care through a comprehensive stepped care approach.

Collaboration and cooperation are the heart of our operation, and you'll be, working closely with clinical and no clinical services, the Administration Intake Worker, and greater team. Together, you'll manage a seamless process from referrals to intake and assessment, guaranteeing swift access to the support our clients deserve.

Moreover, your expertise will extend beyond administrative excellence. You'll be instrumental in the development of case plans and recovery goals, working hand in hand with our dedicated Recovery Coordinators and Peer Advocates. Through your guidance, you'll empower individuals on their unique recovery journeys, fostering resilience and hope every step of the way on the path to wellness.

Your Key Responsibilities

  • Provide specialist advice and guidance to staff to support clients through their service journey.

  • Cooperate with Recovery Coordinators and Peer Advocates to develop holistic, trauma informed case plans and strength-based recovery goals to support clients.

  • Develop positive and effective relationships with a range of key stakeholders including clinical services, community organisations, community housing providers, and other relevant health professionals.

  • Work closely with referral agencies to ensure they have a clear understanding of how the service operates.

  • Participate in clinical supervision, peer development and other opportunities for professional development as required.

Requirements for Success

With a significant background and Allied Health qualifications in Social Work, Mental Health, Psychology and/or Community Services sectors, you will be confident in your understanding of the social determinants of health, trauma informed care, and family sensitive practice.  Other requirements include:

  • Relevant Allied Health degree along with and least 2 years of relevant experience.

  • Understanding of the social determinants of health, trauma informed care, family sensitive practice as well as lived experience and personal recovery frameworks.

  • Demonstrated experience in working with complex clients to develop collaborative care plans and provide recovery goal-directed care.

  • NDIS experience would be desirable.

  • Experience and training in conducting clinical care plans, risk assessments and implementing risk management plans with clients.

  • Excellent organization, time management and problem-solving skills.

  • Familiarity with a dual-diagnosis framework with consumers across the lifespan.

  • Ability to work creatively and safely with clients from diverse backgrounds, including LGBTIQIA+, CALD and Aboriginal and Torres Strait Islanders.

  • Comfortable with ambiguity and juggling competing priorities, and able to work independently and as part of a multi-disciplinary team.

  • Proficiency with electronic health record systems and Microsoft Office suite

  • Full driver licence with no restrictions
