About the Way Back Program:
The Way Back (TWB) Program was first developed by Beyond Blue in 2014, a psychosocial service response to provide psychosocial support for people following suicide attempt or experiencing a suicide crisis. TWB Program is now delivered nationally across most states and territories in Australia.
The opportunity:
This exciting Care Coordinator position is currently open within The Way Back Program team located in the heart of Maroochydore CBD on beautiful Gubbi Gubbi country. In this role you will provide individualised practical psychosocial support to individuals and their families/carers referred to the service by the Hospital and Health services following a suicide attempt or experiencing a suicidal crisis. This is a full-time, max-term contract role ending on June 30, 2026. We welcome applicants from Gympie, with the flexibility to split time working part-time from both our Gympie and Maroochydore offices.
This service is focused on building personal connection, maintaining regular contact with the individual, and supporting them to connect with formal and informal support networks, including integration with relevant community services. Collaboration is key, especially in supporting the individual to address barriers they have identified as contributing to their stressors and distress. Additionally, you will strive to empower the individual by involving them in their recovery process and enhancing their awareness of available supports once they complete the 12-week program.

Responsibilities include:
- Provide non-clinical care and practical psychosocial support for individuals and their families referred to the service for the first 3 months following a suicide attempt or crisis.
- Engage with individuals referred to The Way Back, developing trust and supporting them in a culturally appropriate, trauma-informed, and professional manner.
- Work collaboratively with individuals to develop, implement, and review support plans, including a comprehensive assessment of needs and safety planning.
- Build and maintain strong relationships with the individual, their supports, service providers, hospitals, GPs, and health services.
- Participate in the collection and evaluation of data to monitor and report on program effectiveness.
About you:
You are passionate about fostering engagement with people who are in distress, service providers and the community and are skilled in the ability to develop individual safety plans. We are seeking welcoming, professional, and committed individuals, with the following attributes:
- Possess tertiary qualifications in Mental Health or Social Services related fields
- Demonstrate experience in mental health services.
- Exhibit a dedicated and passionate commitment to teamwork for positive change in the lives of individuals with severe mental illness.
- Showcase the ability to work both independently and collaboratively within a team, ensuring effective delivery of The Way Back service, with exceptional organisational skills.
- Exhibit a willingness to support complex clients dealing with a range of issues.
