About the Way Back Program

The Way Back Program, a psychosocial service response, provides support for individuals following suicide attempts or experiencing suicidal crises. Developed by Beyond Blue in 2014, it is now delivered nationally across most states and territories in Australia.

A full-time Care Coordinator position is available within The Way Back Program team in Maroochydore CBD on Gubbi Gubbi country. As a Care Coordinator, you will provide individualised practical psychosocial support to individuals and their families/carers referred to the service by Hospital and Health services. This role involves working with people who have attempted suicide or are at risk of suicide, providing them with ongoing support and connecting them with relevant community services.

Key responsibilities include:

  • Providing non-clinical care and practical psychosocial support for individuals and their families referred to the service for the first 3 months following a suicide attempt or crisis.
  • Engaging with individuals referred to The Way Back, developing trust and supporting them in a culturally appropriate, trauma-informed, and professional manner.
  • Working collaboratively with individuals to develop, implement, and review support plans, including a comprehensive assessment of needs and safety planning.
  • Building and maintaining strong relationships with the individual, their supports, service providers, hospitals, GPs, and health services.
  • Participating in the collection and evaluation of data to monitor and report on program effectiveness.

About You

To be successful in this role, you will need:

  • Tertiary qualifications in Mental Health or Social Services related fields.
  • Experience in mental health services.
  • A dedicated and passionate commitment to teamwork for positive change in the lives of individuals with severe mental illness.
  • The ability to work both independently and collaboratively within a team, ensuring effective delivery of The Way Back service, with exceptional organisational skills.
  • A willingness to support complex clients dealing with a range of issues.
