The Opportunity: 

The Team Leader, NDIS Services plays an important hands-on role in the planning and development, implementation, review and evaluation of a person’s Individual Person-Centered Plan, whilst also providing supervision, mentoring and development to staff.

Located in Muswellbrook, the role works in Supported Independent Living, Monday to Friday between the hours of 6:00am-8:00pm. 

This role is approximately 30hours of hands on support and 8 hours admin

Paid on Social, Community, Home Care and Disability Services Industry Award, Level 3, Pay Point 1

Role & Responsibilities:  

  • Play a crucial role in ensuring quality supports that foster choice, independence, and goal achievement for participants.
  • Drive Samaritans' Person-Centred Planning processes, overseeing support delivery, maintaining up-to-date documentation, and providing a consistent point of contact for supported individuals and their families.
  • Assist in identifying and assessing client needs, supervise staff, ensure professional and compassionate service delivery, and manage various personal care tasks.
  • Maintain effective communication and relationships with stakeholders, supports clients' living skills development, ensures accurate and timely documentation, reports incidents, and contributes positively to teamwork and achieving outcomes.

About you? 

  • Tertiary qualification in Social Welfare, Social Work, Psychology or Disability Services and/or commensurate experience
  • Demonstrated experience in the delivery of services to people with a disability.
  • Established coordination and problem-solving skills.
  • Demonstrated knowledge and understanding of NDIA planning processes.
  • Ability to think creatively and use initiative, to work independently or as part of a multidisciplinary team.
  • Ability to effectively develop and maintain positive working relationships and networks with government and non-government agencies and services.
  • High level interpersonal skills that support positive and productive relationships.
  • Strong written and computer skills for the creation of plans, programs, reports, correspondence and other
    documents.
