Positive Ageing Officer

As a Positive Ageing Officer, you will provide service navigation support to help connect older people in the community to aged care services.
You will also provide care coordination and navigation to Carers eligible for the Government funded Support for Carers program. The emphasis of the role for the parental leave period will be to:
- Deliver the Care Finder program in the Northern Region with consortium partners
- Provide specialist and intensive assistance to help connect people with relevant supports in the community
- Support people to engage and interact with My Aged Care for screening and eligibility of aged care services and referring for assessment
- Conduct high level check-in with clients and follow up support once services have commenced
- Network with Care Finder partnering Councils and external agencies
- Provide care coordination and navigation support to Carers eligible for Support for Carers funding
- Maintain client records and data to facilitate required monitoring, reporting and evaluation
- Tertiary qualification in allied health or relevant profession (e.g. social work, health/community services or social sciences) and or/ substantial experience working with diverse older people or people with complex support needs
- Demonstrated knowledge of and experience in strengths-based approaches providing information, decision-support, linking, advocacy and casework for older persons with vulnerable profiles and complex needs
- Sound working knowledge and ability to navigate My Aged Care service system
- Understanding of Aged Care reforms and impact on aged care service system in Victoria
- Understanding of Age-Friendly principles and social determinants of health in older age
- A current Victorian Drivers Licence
- A current employee Working with Children Card
