About the role:
Disability Support Coordinators provide direct support to NDIS participants and their families to implement the funded supports in their NDIS plan. Support Coordinators provide specialist NDIS advice to participants and families, as well as other providers, and link them to any necessary mainstream and community supports. A key part of the Support Coordination role is to build the participant's capacity (and that of their informal support network) to implement their own NDIS supports in future.
The Disability Support Coordinator will provide quality customer service to NDIS participants and their families by promptly and proactively addressing the participant's needs and working towards their NDIS goals. Responsibilities include:
Providing advice to the participant about relevant mainstream and community options that may assist them to achieve their NDIS goals. Performing all duties in the interest of positive participant outcomes and the commercial results necessary for the Support Coordination Program to be sustainable. Ensuring that the role is delivered in line with the NDIS guidelines for Support Coordination (e.g. impartial, no advocacy etc.). Negotiating the services to be provided, developing service agreements, creating service bookings, and fulfilling any other administrative requirements of the role. Requesting any assessments necessary to determine the nature and type of funding required (e.g. Home modifications) to meet the participant's needs. Supporting the participant to develop a budget for each support type and regularly reviewing this budget against their NDIS goals. This is a Full-Time Ongoing opportunity based in Footscray, VIC within our Disability Services Program. The position reports to the Coordinator Support Coordination.
Key Selection Criteria:
To be successful in this role, you will have:
Minimum Diploma in Community Services or Disability etc. Tertiary qualifications highly regarded. Experience (minimum 2 years) working with people with disabilities. Demonstrated respect for participants as valued members of their community and understanding their goals and aspirations. High level communication and interpersonal skills with a demonstrated capacity to establish rapport with participants. Proficient in spoken English.
