Summerland Support Services is currently seeking an experienced and well-organised Support Coordinator to join our team on a long-term basis. You'll play a key role in delivering exceptional, tailored support to meet the diverse needs of our NDIS participants.

Key Responsibilities

  • Provide support coordination and/or psychosocial recovery coaching to NDIS participants with varying support needs

  • Use a person centred approach to enhance participants' capacity, choice, and control

  • Collaborate with clients to develop, implement, and monitor individualised support plans

  • Identify and address complex challenges, barriers, and risks alongside stakeholders

  • Demonstrate a solution-focused mindset and encourage innovative thinking

  • Contribute to organisational development and local community networking

  • Adapt to evolving participant needs and circumstances

  • Foster a welcoming, inclusive, and respectful environment for people from diverse backgrounds

Required Qualifications & Experience

  • Minimum 2 years' experience in NDIS Support Coordination or a similar role

  • Minimum Cert IV in Community Services, Disability, or a related health discipline

  • Strong knowledge of NDIS operational guidelines

  • Current driver's licence and access to a safe, comprehensively insured vehicle

  • NDIS Worker Screening Check and Working with Children Check

  • Completion of the NDIS Worker Orientation Module

  • First Aid and CPR certifications

  • Excellent interpersonal and communication skills

  • Strong computer literacy, including proficiency in Microsoft Office

  • Ability to work independently and proactively
