Embrace Willoughby, New South Wales - a modern care home set in a lush, leafy suburb on the Lower North Shore of Sydney. You'll be close to the local community with neighbouring; school, church, parks, Chatswood shopping precinct, local shops, and transport links nearby. It consists of 85 beds, with a variety of care units. Our primary goal is placing our focus on the health, lifestyle, and well-being of our residents.
**The heart of the role**:
As our Care Manager, you will work as part of a team of empathetic and caring clinical professionals. You will support the General Manager in managing and leading the delivery of person-centred care of residents, ensuring the highest standard of ongoing assessment, care planning and evaluation and clinical governance, helping them to live longer, healthier, and happier lives.
**_ The Care Manager role will be offered as a permanent part time position to oversee and support the clinical operations of the Home_**
**Duties include**:
- Oversee the daily clinical operations of the care home
- Support and lead the care home clinical team including coaching, mentoring, staff allocations and delegation, performance management and education
- Review clinical incidents, complaints, and changes
- Lead compliance with AN ACC assessments
- Manage person-centred care planning and evaluation for residents - including case conferencing with families and carers
- Prepare and conduct clinical governance audits
- Oversee medication management in accordance with resident needs and legislative requirements
- Liaise with allied health, medical practitioners, and other services to provide optimal resident care

**What will you bring?**
- Bachelor of Nursing with current APHRA is essential
- Post grad qualification in aged care highly regarded
- Experience managing a minimum of 30 beds
- Proven track record in delivering quality clinical care within an aged care or acute setting
- Excellent clinical assessment skills and care planning capabilities
- Highly effective communication, leadership, and relationship management skills
- An understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes
