Ablepoint Australia is seeking a reliable and compassionate **Support Worker** to assist an NDIS participant with **daily personal care routines** and **community transport**. The participant uses a **manual wheelchair** and requires full physical assistance for transfers and mobility support. A **vehicle is required** for short-distance transport to appointments and community outings.

**Key Responsibilities**:
**Morning support**:

- Assist with getting out of bed
- Showering and dressing
- Preparing breakfast
- Prompting and assisting with medication

**Evening support**:

- Preparing dinner
- Dressing into nightwear
- Continence support
- Assisting into bed safely

**Mobility assistance**:

- Attendant propelling in a wheelchair
- Physical assistance with bed, chair, and vehicle transfers

**Transport support**:

- Drive to local community or health appointments as required
- Ensure safe and comfortable wheelchair transport

**Requirements**:

- Certificate III or IV in Individual Support, Disability, or Aged Care
- Experience in personal care, mobility assistance, and manual handling
- **Valid driver's licence and own vehicle (required)**:

- NDIS Worker Screening Check
- Current First Aid & CPR
- Comfortable working with clients with limited speech and mobility
