Therapeutic Home-Based Care Case Manager

We're looking for a passionate and dedicated professional to join our Therapeutic Foster Care (TFC) program - a specialised service within MacKillop Family Services' Out of Home Care (OOHC) Division, designed to support children and young people who have experienced complex trauma, disrupted attachments, and high support needs.This is not your average case management role.
As part of a small, close-knit therapeutic team, you'll work intensively with carers and children to create meaningful, lasting change.
You'll walk alongside young people as they navigate some of life's biggest challenges - helping them move toward healing, stability, and long-term permanency.This is a full-time, fixed term parental leave position (38 hours per week until 06/07/2026) opportunity based in Preston within our Out of Home Care (OoHC) Program.
The position reports to the Home Based Care Coordinator.Key Selection Criteria:To be successful in this role, you will have:An appropriate tertiary qualification in Psychology, Social Work or related discipline is essential.
