**Your new company**

Our clients, based in West Lakes, are looking for a Community Aged Care Coordinator to join their team on a temporary basis. This role is an 8-month contract, with a potential extension. The main duties and responsibilities would entail providing high-quality care and support to clients and customers in the aged care sector.

**You will be responsible for**:

- Evaluating, referring, prioritising, managing cases, and coordinating services within a community services context.
- Collaboratively creating care plans based on client objectives, preferences, and needs, aligning with relevant aged care funding guidelines, legal requirements, and organisational instructions.
- Recognising gaps in client care and service delivery.
- Support for Diverse Backgrounds: Assisting individuals from Cultural & Linguistically Diverse (CALD) backgrounds.
- Using electronic consumer management systems and reporting tools.
- Skilfully handling competing priorities to achieve high-quality outcomes within agreed timeframes.
- Promptly identifying risks and escalating as needed.
- Providing support to carers and client advocates.
- Familiarity with the My Aged Care system and portal.
- Offering guidance on advanced care planning and end-of-life care decisions

**What you'll need to succeed**
- Minimum qualification of diploma in Human Services / Health / Community / Disability or equivalent, or extensive experience working in a similar role (essential)
- Current National Police Clearance (essential)
- Current First Aid Certificate (desirable)
- Current SA Driving Licence (essential)
- Reliable vehicle and comprehensive vehicle insurance (essential)
- Current DHS NDIS Worker Screening Check (essential)
- Current Covid-19 Vaccinations (2 doses + 1 booster)
- Experience working in a similar fast-paced community-orientated role
- Experience in individual assessment, referral, triage, case management and coordination within a community services environment (preferably aged care)
- Experience in co-designing Care Plans according to client goals, preferences, needs and requests, and in line with relevant aged care funding guidelines, legislative requirements, and organisational work instructions
- Experience of identifying gaps in both client care and service provision
