As a Social Worker (AHP2), you'll deliver high-quality, evidence-based psychosocial care within acute, subacute, mental health, and community health settings.
Working within a Single Service, Multiple Site model, you will actively contribute to a multidisciplinary team focused on holistic, person-centred care.
In this varied and rewarding role, you'll be responsible for comprehensive psychosocial assessments, clinical interventions, discharge planning, and community liaison.
You'll also contribute to teaching, mentoring, and service development activities that drive best practice.

About You You are a compassionate and capable social worker with a strong commitment to patient advocacy, teamwork, and continuous professional growth.
