Go back Midlands Partnership NHS Foundation TrustLevel 1 Social WorkerThe closing date is 31 August 2025An exciting opportunity has arisen within Lichfield & Tamworth Community D2A Team for a Level 1 Registered Social Worker Band 5.You will be part of the South Discharge to Assess Service and will work closely with Multi-Disciplinary team members including Social Workers, Social Care Assessors, Nurses, Therapists and the Home First team.You will be responsible for supporting the delivery of the personalisation and social inclusion agenda, in line with the principles of the Care Act 2014.

The relevant SWLA stage (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration.
