Thrive Services is seeking a compassionate and experienced **Family Case Worker** to join our **Targeted Earlier Intervention (TEI)** team at our Lithgow office.

We offer two specialised programs - Targeted Early Intervention & Specialist Homelessness Service. We work from a person-led and strength-based approach, which involves a holistic, goal-orientated and coordinated approach to addressing the needs of families experiencing challenges. Our focus is on early intervention and prevention support that aims to strengthen families and enhance life opportunities for children and their families. **About the role**
Hours: Part-Time | 21 hours/week (Mon, Tues & Wed)
Award: SCHADS 2010 Level 4
Location: Lithgow NSW
As a TEI Family Case Worker, you will provide tailored case management, parenting programs, and group work to families with children under 18 years old who are living in the Lithgow LGA. You will work collaboratively with families to develop and implement individualised case plans, with the focus on enhancing family functioning, support with improving family relationships and connections, and building capacity of families to address their own needs.
**What you bring**:
- Minimum requirement of a tertiary qualification in community services, or other relevant areas and at least 3 years relevant case management experience.
- Demonstrated experience in providing holistic case management support focusing on strengths and needs, including working collaboratively with clients to develop tailored action plans, helping them achieve their goals.
- Knowledge and experience in planning and facilitation of evidence-based programs/group work and activities within the community.
- Strong communication, interpersonal, organisational & problem-solving skills, and the ability to work independently and as part of a team.
- Engage in strength-based, reflective practice with dedication and enthusiasm, demonstrating a passion for supporting families and the local community.
- Must have a current working with children check number.
- Current NSW driver's licence and reliable vehicle.
- Current First Aid Certificate (or willingness to obtain).
