About the role
PAC is currently seeking a driven and experienced Care Manager at our Thornleigh facility. Reporting to the Facility Manager, you will lead the coordination of all clinical care documentation processes, activities and services. You will work with and mentor our current Care Manager then cover her long service leave.

Key responsibilities
- Provide direction and supervision of Staff training, education and care delivery supervision.
- Assessment and documentation of resident health care needs, and the development and implementation of all nursing care plans, and maintenance of their care and medical records.
- Consult and coordinate with health care team members to assess, plan, implement and evaluate resident care plans.
- Maintain a clinical care documentation portfolio that ensures that quality care outcomes to residents are delivered in a timely and appropriate manner.
- Maintenance of staff rosters in accordance with benchmark hours.

Essential Criteria
- Knowledge and understanding of the relevant legislation relating to clinical care requirements and documentation, and in particular, ANACC and DHA requirements.
- Knowledge of issues, and contemporary practices affecting the delivery of quality clinical services in residential aged care facilities.
- Registered Nurse - APRHA Registration: Maintain current unencumbered APHRA registration
- Post graduate qualifications in health care management (desirable)
- Knowledge, skills and understanding of Dementia in older people
- Excellent Computer skills (Microsoft Office, Excel, and Outlook)
- Excellent verbal and written communication skills.
