About the opportunity

We are currently looking for an experienced Care Advisor to support and grow our home care services in the (location) Region. Working closely with a supportive regional team, the position will be responsible for developing effective relationships to expand the home care services program in their local region as well as developing and maintaining a caseload of clients which will involve:
Conducting home consultations.- Developing and maintaining goal directed care plans.- Monitoring and reviewing client services and coordinating support services.- Managing client expenditure.

To thrive in this role, you will have:
- Passion in helping people in your community, with strong relationship building, time management and budget management skills.- Previous case management experience as well as relevant qualifications.- Current Victorian Drivers Licence.- Strong communication and organisational skills.- A safe, sensible approach with a practical understanding of people's physical limitations.- The ability to be flexible and adapt to meet clients' individual needs.
You will also be required to provide evidence of, or in the process of obtaining:
- National Disability Insurance Scheme (NDIS) Clearance.- First Aid Certificate that includes CPR.- Proof of current influenza vaccination.
