**_Due to continued growth at My Flex Disability we are currently seeking a full-time Disability Services Coordinator. _**

You will be a part of our community team to deliver exceptional individual support to maintain a positive experience for our disability clients. Our primary objective is to enable clients to realise their personal goals, to enhance their life choices and maintain their independence and wellbeing.

**This is a full time 8:00am to 4:00pm Monday to Friday position based in Perth**.

**Duties and Responsibilities**:

- Coordination and development of supports to a caseload of NDIS participants living in the community
- Establish and build strong relationships with clients and their families in the provision of personalised disability services
- Seek new opportunities to grow your client database and maintain a strong network of business referrals
- Conduct client assessments and regular reviews with a focus on personalised support in line with NDIS standards
- Completing NDIS quotes, claims and reporting as required
- Undertake effective leadership of support workers where required
- Liaise with the Schedulers, Homecare and Nursing teams for provision of appropriate and timely client services
- Actively participate in continuous improvement in delivery of services and development of processes
- Maintain client records accurately and confidentially
- Assist with administration activities as required under direction of the Disability Services Manager

**Experience, skills and requirements**:

- Knowledge and understanding of NDIS funding and the NDIS portal
- Experience in planning and coordinating meetings with clients and key stakeholders.
- Demonstrated experience in the in the community disability sector is essential
- Previous experience in coordination/case management with strong client focus
- A pro-active approach in assisting clients to reach their full potential
- Strong written and verbal communication skills
- Effective problem solving and negotiation skills
- NDIS Worker Screening Check (or willing to get)
- Working with Children Check (or willing to get)
- Current First Aid (or willing to get)
- Current WA drivers license
- Covid-19 Vaccination evidence
