Our Youth Case Manager will provide targeted and trauma informed, early intervention case management support to young people aged between 12-24yo and their families in our Oasis OLIY program.

WHAT YOU WILL BE DOING
- No two days are the same when working with our young people.
- Based in Rockdale, you will experience the wonders of the young people today and support them to overcome the barriers that exist for them. We do this using a trauma informed approach to ensure that our young people are at the centre of everything we do.
- You will provide 1:1 case management support to young people aged between 12-24years old in the Georges River and Bayside areas. We work with young people to identify their goals and support the steps that need to be taken to get there.
- Using a range of therapeutic and evidence based methods, we aid in developing and implementing SMART case plans.
- A big part of what we do is collaborating with the community and the supports that already exist to support our amazing young people. We strongly advocate for our young people and promote self-efficacy, empowerment and capacity building.
HOURS AND SALARY
Full-time position, Monday-Friday, 38hours/week.
Salary and conditions of employment are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, level 4.
SUCCESSFUL APPLICANT WILL HAVE- Tertiary or other qualifications within Community Services, Social Work or similar discipline- Experience in the implementation of case management and appropriate intervention strategies for working with disadvantaged and/or complex needs clients.- 1 year work experience working with vulnerable and at risk groups- Demonstrated experience in working with young people and their families- Established knowledge & understanding of barriers faced by young people including homelessness, mental health, addiction and appropriate intervention strategies- A current NSW Drivers License- Valid NSW Working with Children Check and willingness to undergo National Police Check- Ability to provide proof of Eligibility to Work in Australia- Ability to provide proof of Vaccination against COVID-19 (at least two doses)
