We are the main provider of health services to people living in the inner southeast suburbs of Melbourne, offering ambulatory, inpatient, and community-based services.Position Details:Permanent Full Time - 1.0 EFT (or 80 hours per fortnight)Work schedule includes weekends, Tuesday to Saturday (most weekends required, negotiable)Location: Cardiac Heart Failure and Cardiology - Alfred HospitalAbout the Department:The Social Work Department at Alfred Health focuses on quality of life and participation in care, addressing psychosocial impacts of illness and hospitalisation, and supporting patients and families through transitions from hospital to home.The Role:The Social Worker will provide advanced social work services within the Alfred Heart Transplant and Cardiac Heart Failure Service, including comprehensive assessments using the Stanford Integrated Psychosocial Assessment for Transplant (SIPAT), and support within the Trauma stream.
Flexibility to work across multiple campuses may be required.Essential Skills and Qualifications:Eligibility for membership of the Australian Association of Social WorkersBachelor of Social Work or recognized equivalentEffective team collaboration and family engagement skillsExperience or knowledge in Family Violence, Child Protection, NDIS, or related areasKnowledge of hospital social work legislation and policiesDesirable:Postgraduate qualifications in Social Work or related fieldsHospital-based experienceProficient IT skillsBenefits:Salary Packaging and Novated LeasingDiscounted Health InsuranceStaff Car Parking (subject to availability)Onsite Gym at Alfred HospitalClose to shops and public transportFor more details, download the attached Position Description.
