Position Details:
Permanent Full Time - 1.0 EFT (or 80 hours per fortnight)
Work schedule includes weekends, Tuesday to Saturday (most weekends required, negotiable)
Location: Cardiac Heart Failure and Cardiology - Alfred Hospital
About the Department:
The Social Work Department at Alfred Health focuses on quality of life and participation in care, addressing psychosocial impacts of illness and hospitalisation, and supporting patients and families through transitions from hospital to home.
The Role:
The Social Worker will provide advanced social work services within the Alfred Heart Transplant and Cardiac Heart Failure Service, including comprehensive assessments using the Stanford Integrated Psychosocial Assessment for Transplant (SIPAT), and support within the Trauma stream. Flexibility to work across multiple campuses may be required.
Essential Skills and Qualifications:
Eligibility for membership of the Australian Association of Social Workers
Bachelor of Social Work or recognized equivalent
Effective team collaboration and family engagement skills
Experience or knowledge in Family Violence, Child Protection, NDIS, or related areas
Knowledge of hospital social work legislation and policies
Desirable:
Postgraduate qualifications in Social Work or related fields
Hospital-based experience
Proficient IT skills
