About the role

We are seeking a values-driven Case Worker to provide effective service delivery, ensuring case management, assessment and referral, education and support are provided in accordance with TSA guidelines.

You will successfully
- Provide efficient day to day service delivery through education, interventions, group work, assessment, referral and case work within a recovery-orientated and person-led service environment
- To develop and monitor individual treatment plans and maintain case management documentation, records, reports and data collection, utilising established case planning procedures
- Undertake ad hoc duties as requested by the Manager, or Team Leader
- Demonstrate commitment to an environment that values continuous improvement and innovation in the delivery of high-quality services
- Acquire and maintain a good working knowledge of referral and support options for participants
- Work collaboratively and professionally with other services
- Participate in sector events and training

This is a full time permanent role based in Dooralong, NSW. Salary and conditions are in accordance with the Social, Community, Home Care and Disability Services Award Level 4.

You will have
- Minimum of Diploma level qualification in a relevant discipline (AOD, Community Services, Case Management, Mental Health)
- Completed 4 AOD Skill Set Units (or willing to undertake/complete during employment)
- Knowledge of AOD, mental health, community services and health sectors.
- Demonstrated experience and ability to work with those impacted by substance abuse, gambling and mental health conditions
- Experience providing brief intervention and harm minimization information and support to individuals
- Experience working with people from Aboriginal and Torres Strait Islander background
- Experience working with people from culturally and linguistically diverse backgrounds and religions
- Experience working in collaboration with other organisations
- Current Drivers licence
- Demonstrated ability to deal with unpredictable situations and behaviours utilising de-escalation techniques.includes an alcohol, gambling and drug free workplace
- Demonstrated strong administrative, time-management and communication skills
