The Case Manager is responsible for:

- Supporting the effective transition of all new clients into MLC in a timely, yet safe manner
- Acting as the primary contact point for both internal and external stakeholders throughout the process of transition
- Ensuring the Executive Leadership Team receive timely and relevant information pertaining to transitions to ensure that performance targets are being met.

**As the Case Manager, you will**:

- Project manage clients transition into MLC safely, effectively, and within appropriate timeframes.
- Oversee the multidisciplinary MLC team in facilitating the transition including: Clinical Team members, Property and Procurement, Finance and Risk, Operations, Human Resources, and Rostering
- Build strong working relationships with clients, their families/guardians, support coordinators, plan managers, referrers, and the hospitals etc to ensure that MLC's reputation remains one of professionalism, transparency, trustworthy, and proactive.
- Lead weekly (or more frequent) Transition Meetings with all key internal stakeholders to ensure that transition tasks are completed on time, to MLC's standards, and in line with the client's goals.
- Maintain strong oversight over all Transition Project Trackers, ensuring all tasks are completed on time and prior to transition, and that persons are held accountable for their actions.
- Identify, escalate, and support the underperformance of team members or issues in transition to ensure that the uphold MLC's standards.
- Identify process improvement opportunities to further streamline and enhance the Transition process to enable MLC to achieve its growth and scale objectives.
- Facilitate Transition Reviews to identify opportunities to improve our Transition process.
- Allied Health Applicants: Must hold a Bachelor or post graduate degree In an Allied Health Profession. (Ideally Occupational Therapy, Social Work, Physiotherapy)
- Strong project management skills with a high degree of drive and passion for patient centric care.
- Minimum of three years' experience managing large, diverse care teams in clinical and home care settings
- Experience in launching new, and integrating acquired, services
- Experience managing geographically dispersed (multi-state) operations
- Demonstrated understanding of, and adherence to, safety and quality standards, work, health and safety (WH&S), NDIS standards and the positive client experience.
- Progressive, adaptable, and flexible with ability to successfully introduce change and provide responsive services to meet clients' needs
- A strong focus on person centred, exceptional care and a commitment to quality client outcomes - attention to the small details that matter to our clients
- Excellent communication skills
- Demonstrated ability to create a culture of operational excellence (quality, efficiency and consistency) and learning
- Confident in managing risk through sound clinical governance and risk management practices

**Please note prior to commencement successful applicant will be required to**:

- Obtain an Occupational Medicine Compliance Certificate relating to assessment, screening & vaccination processes against specified infectious diseases including Covid-19
- Undergo a pre-employment National Police Check
- Obtain an NDIS worker screening clearance and show evidence of completion of the NDIS Worker Orientation Module.
