Our modern social and affordablehousing features 84 one-bedroom villas for seniors over 55, over 45 for Aboriginal and Torres Strait Islander peoples, and 38 two-bedroom townhouses for single-parent families.Our team works together to provide secure, high-quality accommodation with integrated services that empower residents to live life their way.As a Tailored Support Coordinator, you will provide effective assessment and planning to coordinate and support tenants and household members access to a range of individualised services to improve their wellbeing and achieve their goals in life.The successful applicant will have:Certificate IV – Social Work or Community ServicesExperience in Social Housing, Community Services or related disciplineDemonstrated ability to facilitate the provision of effective and individualised support to achieve positive outcomes for tenants and household membersExperience working with high need, complex clientsHigh level negotiation skills and ability to liaise with key stakeholdersCompetence in service assessment and documentation (including service planning)Demonstrate an understanding and appreciation of BaptistCare's Christian identity and promote the organisation's values through interactions at workTeam player (cross functional team)Good working knowledge of Work Health and Safety practiceComputer literate - intermediate to advanced MS Office and OutlookGood written and oral communication skillsTenacity, tolerance, empathy and patience with tenants and their needsFlexible and willing to work between locations as requiredWilling to take part in ongoing educationA Valid NSW Driver's LicenseNSW Working with Children Check for paid employment purposesA current First Aid Certificate or willing to obtainIf you're passionate about community support and want to be part of a team that values respect, inclusion, and empowerment, we'd love to hear from you
