Our clients are all very individual and have varying needs.
For this role, we are seeking an individual experienced with complex high-care needs including behaviours.
Customers have a wide range of professional supports and full training in their care plans is provided.

Proposed Roster - PM shifts - 41.75 hrs.pdf Key duties include: Delivering a wide range of supports assisting people with disability and mental health needs as they move from hospital to home; Supporting customers to engage in community-based activities; Supporting person-centred plans that identify a customer's life goals; Ensuring positive behaviour support plans are followed; and Administration duties, expense and mandatory reports.
What you'll need?
Extensive experience in supporting customers with behavioural support plans; Extensive experience working with challenging behaviours- essential; Previous experience with manual handling and medication administration; Significant personal care and domestic duties experience; Ability to manage situations as they arise with patience and maturity; Certificate in disability or mental health or higher; A Valid drivers licence and a willingness to use your private vehicle (aligned with Activ's motor vehicle allowance rebate).
