HammondCare provides care primarily through hospitals, residential care homes and home-based care as well as services for dementia support, community palliative care, rehabilitation, positive aging and older persons' mental health.
About The Role
Reporting to the Head of Pastoral Care, the Pastoral Care Coordinator provides pastoral care, leadership and support to clients, their families, team members and pastoral care volunteers. The role supports clients, who are in HammondCare's palliative care service, by visiting them in their own homes across the Northshore.
The role requires access to a car (mileage reimbursed), use of a pool vehicle may be available. The role is permanent part-time working 3 days per week (22.8 hours pw) (one day must include a Wednesday).

Responsibilities
Provide pastoral care and support to clients, relevant families and staff of HammondCareBe the point of contact for relevant internal and external support/referral servicesAct as a role model for staff and as an advocate for HammondCare's Mission, Motivation and Mission in Action, and its Christian valuesShare pastoral expertise and coach staff to support the spiritual needs of clientsParticipate in multidisciplinary team meetings and make a contribution about the client's emotional/spiritual needs and goals and contribute to the development and ongoing evaluation of a care planFacilitate regular carer support groupsNetwork with other religious leaders and services and coordinate relevant care, resources and group activitiesStrategically develop tailored resources, ideas and activities to enhance the spiritual and emotional wellbeing of clients and families, collaborating with management, staff and volunteersCommunicate and build trust by developing relationships with staff and volunteers.Liaise with managers, case managers and attend staff and case management meetings as requiredTraining staff on spiritual care and wellbeingLeading and training a team of pastoral care volunteersWorking and collaborating in a Pastoral Care team
About You
Commitment to engage and align with HammondCare's Christian Mission, Motivation and Mission in ActionStrong understanding and experience of people living at home with life-limiting illness including dementia, the impact on carers, and journeys of grief and bereavementExperience in palliative care - desiredStrong personal and active Christian faith and church involvementTheological /Pastoral Care qualifications and experienceDemonstrated capacity to pastorally care for and enhance the wellbeing of those in needHighly developed communication skills including listening, compassion and empathyStrong administrative and computer skillsAccess to a car, Australian's driver licence and comprehensive car insurance
