**JOB TITLE**:

- Support Worker_

**JOB DESCRIPTION**:

- The Disability Support Worker is responsible for the provision of person/ family centered support services that empower people with disability to achieve their potential and live the life they choose. Disability Support Worker works under limited supervision with readily available assistance._
- Employees need to demonstrate a Person-Centered and positive approach to supporting and empowering customers._

**LOCATION**:

- Queanbeyan NSW_

**MAIN DUTIES / RESPONSIBILITIES**:

- _Support clients to participate in age appropriate activities and tasks including employment, in home support and community access._
- _Support clients to access activities and outings in the community_
- _Provide direct care assistance to each client in accordance with their service plan and as directed by the Manager_
- _Work in accordance with the medication administration policy, work health and safety procedures and client right's policies including privacy and confidentiality._
- _Monitor the safety and well-being of the clients and report any concerns to the Manager._
- _Provide 1:1 or small group training and support in response to individual client needs._
- _Ensure completion of appropriate documentation. Relevant information on support provided is to be recorded in client notes and communication book as required._

**QUALIFICATIONS/ REQUIREMENTS**:

- _NDIS Worker Orientation Module_
- _New Worker NDIS Induction Module_
- _NDIS Worker Screening_
- _Valid Driver's License_
- _Working with Children Check_
- _National Criminal Record Clearance_
- _Certificate III in Disability or other relevant qualification or working towards obtaining qualification within 12 months (Desirable)_

**EXPERIENCE**:

- _Experience in the delivery of personal care services and/or challenging behaviors is viewed favorably_
- _Must have experience in mental health_

**SKILLS**:

- _Genuine desire to provide the best quality support for people with disabilities._
- _Reliable character._
- _Well-developed communication and interpersonal skills including the ability to work and communicate effectively with clients, their families, community agencies and other professionals._
- _Ability to work in a very busy environment and to work both independently and as a part of a team._
- _Understanding of privacy and confidentiality issues._

**Job Type**: Casual

**Salary**: $30.00 - $60.00 per hour
