**This new opportunity is for a Social Worker / Case Manager**:
Our client, WAYS Youth & Family, is looking for a Social Worker / Case Manager who is passionate about working in an organisation that provides quality outcomes to young people, adults, and their families in the community.

We are an established not-for-profit community organisation whose mission is to work with young people, adults, and families. We provide a range of social services through education, training, case management, counseling, outreach, and more.

**About the role**
- Developing effective case plans for individuals and/or their families, who may be experiencing substance misuse, homelessness, mental health concerns, or barriers to education.
- Ensure clients have access to a range of supports and services in relation to health, housing, employment, and education.
- Support people to develop stronger links to family and community.
- Contribute to community engagement, music events/programs, psychoeducation/therapeutic workshops, school forums, outreach, and service promotion activities.
- Responsible for developing, executing, and managing an effective and targeted outreach program designed to increase the number of young people accessing the WAYS Wellness Centre and other WAYS programs.

**About You**
- Demonstrated experience in providing social work and case management services for disadvantaged people and their families addressing a range of mental health, education, homelessness, and drug and alcohol issues.
- Relevant tertiary qualifications in Social Work, Psychology, or equivalent.
- An enthusiastic attitude to connect, engage and support people
- An understanding of case management methodology, models, and systems.
- You have a NSW Working with Children Check (or willingness to obtain)
- Ability to work independently, manage time and multiple demands.
