- We are seeking Care Coordinator who is motivated, reliable, and passionate about aged care.
- Delivering services under a range of funding sources including private pay clients, My Aged Care and NDIS, the primary responsibility is to provide quality in home care and assistance to our fast-growing client base.
- The successful applicant will be working with our care team, liaising with Clients, Family Members, Staff and a wide range of Stakeholders to ensure services are provided in accordance with client's needs, goals and preferences.
- Assessment, planning, and reviewing care plans as required, services are Client focused and Consumer always directed with quality outcomes achieved using a flexible, responsive, creative and collaborative approach.
- In addition to case management, this role will manage inside and outside sales enquiries, develop and maintain new and existing referral sources.
**Qualifications**:
- The Care Coordinator will be responsible for:
- Building strong connections with clients and their families
- Assigning staff to clients based on staff qualifications and availability, as well as client preferences
- Organize support to clients with disabilities and monitoring and reporting on their progress
- Referring clients to agencies that can provide additional help
- Assessing client needs and resources for health, welfare and other facilities and services
- Maintaining accurate records of incidents and feedback as required
- Re-organizing work priorities to meet the needs of a fast-paced environment
- Supporting all clients to meet their goals
- Conducting Client Care Assessments and Care Planning
- Supervision and management of Care Staff
- Discussing and promoting Right at Homes services with medical professions and referral sources
- Reporting to head office- Ability to work in a fast-paced environment
**Preferred Skills**:
- Experienced in assessing and planning care needs
- Sound understanding of the Aged Care and Disability systems
- A strong client focus which is based on their goals, preferences and needs
- Demonstrated experience managing staff
- Current National Police Check
- Working with Children Check
- Full Driver's License
- Organizational, administrative and time management skills
