At Murray Mallee Aged Care Group (trading as My Choice Care), we know that behind every great client interaction is an even greater Care Partner - the person who transforms a list of services into meaningful moments.
Based in our Fullarton office , you'll be a champion of client support, ensuring dignity and independence for seniors across our community.
Your Day As a Care Partner Will Include: Conducting Client Assessments: Visit clients in their homes to understand their unique needs and goals.
Creating Personalised Care Plans: Design service packages that fit like a glove.
Reviewing Client Progress: Read support worker notes like a detective, spotting opportunities to enhance independence.
Organising Services: Coordinate everything from meal deliveries to mobility aids, ensuring each piece fits the bigger picture.
Managing Equipment Purchases: Help clients get the tools they need - from grab rails to bathmats.
Regular Plan Reviews: Keep care plans fresh and relevant, adjusting as clients' needs change.
What We're Looking For: Certificate III in Community Services (or equivalent experience with a willingness to learn).
Current SA Driver's Licence and reliable vehicle.
National Police Certificate.
Understanding of care or community services.
Cultural awareness and sensitivity.
