**Description**:
About the Role

It is the role of the Aboriginal Health Worker to contribute, where possible, to ATSICHS comprehensive health support services to clients with the aim to achieve improved health outcomes for all Aboriginal and Torres Strait Islander people. This includes effective triage, prevention, education, support and recalls.

Duties
- Explain procedures and treatment to clients to gain cooperation and understanding, and allow clients to make informed decisions about their health management.
- Contribute to the effective management of patient care plan systems, including participating in patient care conferences, managing patient follow-up procedure.
- Identify care needs and document patient consultation in patient's medical file.
- Assist Doctors and other health professionals with communication, treatment and any advice relevant to a client's treatment.
- Assist in the provision of high quality emergency care that at times is independent but with support from other health professionals.

**Skills and Experiences**:
Please note that it is a genuine requirement that this position be filled by an Aboriginal or Torres Strait Islander person in accordance with s25 of the Anti-discrimination Act 1991 (Qld).

Covid-19 requirements
- Immunisation history showing you have received two doses of a Covid 19 vaccine.

Required

Certificate III in Aboriginal Primary Health Care (Clinical Practice) or higher.
Previous demonstrable experience in relevant Aboriginal Health Worker field.
Demonstrated ability to meet targets and performance outcomes.
Ability to support a continuous improvement model for achieving outcomes.
Decisive listening, influencing and persuasive skills.
Medicare Provider Number as an Aboriginal health Practitioner (non-restricted).
Full un-restricted AHPRA Registration.
Working with Children's Card (Blue Card) or be eligible to apply.
Disability Worker Screening Care (Yellow Card) or be eligible to apply.
Current National Police Check.

Preferred

Minimum two (2) years experience in a Primary Health setting.
Previous experience of working with Aboriginal and Torres Strait Islander clients.
Previous experience in a multidisciplinary health team.
- Previous experience in chronic disease management.
