You will work within residential and community transitional care settings and home care programs; collaborating closely with families, healthcare professionals, and the Allied Health team to make a meaningful impact on the aging journey of our clients.
As part of the LHG multidisciplinary health team, you'll also participate in health promotion activities across all LHG services.What You'll DoConduct thorough Occupational Therapy assessments on admission and as required to create tailored therapy plansPerform Home Safety assessments and recommend home modifications to support independent livingPrescribe assistive technology and equipment to enable clients to live comfortably and confidently at homeCollaborate with clients, families, stakeholders, and Allied Health Professionals to address therapy needsDeliver evidence-based therapy to help clients maintain or improve their daily living abilitiesWork alongside the Allied Health Team to support wellness and reablement program in both residential and community care environmentsWhat You'll Bring:Demonstrated experience applying effective treatment and evaluation techniques in an Occupational Therapy settingExcellent interpersonal skills with the ability to work collaboratively across multidisciplinary teamsStrong communication and documentation skills, with a client-centered approach to careAbility to manage a varied caseload and adapt to changing prioritiesA passion for supporting older adults to achieve their highest level of independenceSpecial Requirements:Bachelor's degree in Occupational Therapy with current AHPRA registration – essentialExperience in community or rehabilitation settings for older adults – highly regardedCurrent South Australian driver's licence – required for travel across LHG sitesNDIS Worker Screening Clearance – required (NDIS risk-assessed role)Willingness to undertake a pre-employment medical assessmentRecent graduates are welcome and encouraged to applyIn line with aged care health guidelines, all recommended vaccinations are encouragedWhy Join LHG?
