Visit us today and check out to see why more people from across the globe are moving here.About the OpportunityThe Health Services Lead, Primary Health Care is a member of the Primary Health Care leadership team and is responsible for advancing priorities of the portfolio via supporting the ongoing development and implementation of assigned Primary Health Care, Wellness, Chronic Disease Management, Department of Family Practice programs services and initiatives.
The Health Services Lead works together with other members of the Primary Health Care leadership team to implement multiple Primary Health Care projects and initiatives to support advancement of patient centered, community based primary health care programs and services within their Zone of Nova Scotia Health.Presently, they are responsible for day to day operations of our chronic disease teams, with a focus on Diabetes Centres and the wellness portfolio across Eastern Zone, spanning multiple sites.
The Health Services Lead will work closely with all interprofessional teams to manage performance standards and operational requirements by facilitating the development and implementation of consistent Primary Health Care, Chronic Disease management and organizational priorities, while respecting uniqueness and individuality of teams and communities.
The Health Services Lead will work closely with interprofessional teams to manage operational requirements, performance standards and support quality improvements.The Health Services Lead is knowledgeable of provincial and zone governance structures, community-based agencies and organizations, and the community based collaborative team approach to care.
The Health Services Lead is responsible for developing and maintaining strong working relationships with health care staff, other Zone based NSH departments and teams, external individuals, and partner agencies.
Integrated Chronic Disease Management and Wellness portfolio are under review and management functions and areas of focus may evolve.About YouWe would love to hear from you if you have the following:Baccalaureate degree from a health discipline or related fieldMinimum of three (3) to five (5) years' recent and related progressive leadership experience in a health care setting.Experience in chronic disease management and primary health care, specifically diabetes management will be considered an asset.Experience working with physicians, nurse practitioners and registered nurses, among other health professionals an assetCurrent registration with professional association (if applicable)Knowledge of evidence-based models of care for chronic disease management and wellnessDemonstrated knowledge of primary health care, health promotion, health determinants, and dynamics of health needs in communityStrong interpersonal and communication (oral, written and presentation) skills including the ability to work collaboratively with various professional and community individuals, demonstrating a high level of judgement, tact, creativity, diplomacy, discretion, and confidentiality.Demonstrated experience with budget and resource managementDemonstrated experience in program development, project management, implementation, monitoring and evaluation in community settingsDemonstrated ability to work with interdisciplinary teams, physicians and other stakeholders across multiple governance structures to establish and sustain strong collaborative family practice teamsDemonstrated ability to build and maintain effective collaborative relationships and partnerships with stakeholders and clientsStrong organizational, time management, and problem solving skillsDemonstrated experience in quality improvement and change management initiativesDemonstrated self-motivation, self-direction, leadership, team development, organizational and facilitation skills.Strong human resource management and financial management skillsDemonstrated regular attendance in current and previous employment.Demonstrated commitment to diversity, equity and inclusionExperience in a unionized environment will be considered an assetCompetencies in other languages an asset, French preferredPlease ensure your resume is up to date and includes all relevant education, experience, training, and certificationsHoursPermanent full-time position; 75 hours bi-weeklyShifts include days, nights and weekends (or other specific hours of shifts)Travel to various sites may be requiredCompensation and Benefits$40.95 - $54.42 HourlySuccessful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.Once You've AppliedThank you for your interest in this position.
