**About the role**:
We are seeking an individual to join our team based in our ANZAC Village in Sydney's stunning Northern Beaches. As a **Case Manager**for our **Homes for Heroes**program, you will be a primary point of contact for intake and referrals, you will collaborate closely with the Veteran Housing Program Manager and those in peer positions in providing a critical service to veterans who are experiencing homelessness or at risk of becoming homeless.

This role is instrumental in advancing Homes for Heroes vision, empowering veterans to break the cycle of homelessness. Your focus will be on supporting each Veteran with long-term housing goals by working closely with them and a range of providers to create a Transitional Housing Support Plan with clear objectives and goals to achieve this outcome.

**A bit about you**:

- Relevant qualifications in health, welfare, community services, or a mental health discipline
- Experience in case management, supporting clients with complex needs in a trauma-informed, recovery-oriented approach
- Minimum 5 years' experience in client focused roles
- Knowledge of the Australian Defence, ESO, and Veteran communities, with the ability to navigate relevant agencies and departments
- Familiarity with the homelessness sector and housing services in NSW
- Demonstrated ability to create and implement Support Plans based on SMART goals
- Proven ability to work under pressure, handle difficult situations, and manage a diverse caseload effectively
- Willingness to work occasional weekends or evenings and attend work related events
