**We are looking for a budding Support Coordinator to join our enthusiastic and growing Support Coordination team.**

**About the role**
If you are passionate about assisting individuals and/or their families build capacity in self-advocacy and creating individual goals and outcomes in collaboration with a variety of stakeholders, then we want to hear from you
Reporting to the Senior Social Worker, this position will be responsible for helping individuals, families, groups and communities to enhance their individual and collective well-being. It aims to help people develop their skills and their ability to use their own resources and those of the community to resolve problems. The position will work closely with the key roles across the multidisciplinary team and will liaise with key internal and external stakeholders.

**What you'll need to succeed**
- Knowledge of, or experience in, the disability sector;
- Experience in the delivery of services to individuals and/or their families;
- Strong communication skills;
- A demonstrated family and person-centered approach;
- Good computer literacy and experience working with a CRM;
- Satisfactory DHS Working with Children Check, NDIS Worker Screening Clearance, and National Police Check; and
- All mandated doses of COVID-19 vaccinations.
Highly desirable
- Tertiary qualifications relating to Disability or Case Management.
- Experience in the provision of case management and working collaboratively with families and services providers.
