Westside Lodge Dual Diagnosis Rehabilitation Centre (DDRC) provides a supportive and safe inpatient environment over a three month period, for people aged 18 years and over requiring a client-centred, recovery oriented Dual Diagnosis rehabilitaiton program. The therapeutic and educational program offered will support and promote positive lifestyle choices. Linkages to post rehabilitaiton community based programs and services are alos a key component of the DDRC program. The DDRC Intake and Assessment Coordinator is a key role within the DDRC, coordinating all intake, assessment and admission management for the DDRC.
The role includes receiving assessments from service providers and collecting additional information to inform treatment planning and timely admission to WSL DDRC; completion of screenng and assessment tools; and in collaboration with community service providers, facilitate treatment plans that support clients prior, during and post admission. The Intake and Assessment Clinician will work closely with the DDRC Social Worker to make assertivelinkages to support client admission into and discharge from WSL DDRC.
