**About the Role**
The Care Coordinator will assist with the provision of quality services through assessment, case management, monitoring and review of services for eligible clients in accordance with McLean Care Ltd values and policies.
Reporting to theClinical Care ManagerHome and Community Care Services NSW, you will possess the following:
- Current APHRA registration as an Enrolled Nurse
- Ability to balance competing priorities of individual's expectations against resource levels of the program and communicate these to clients
- High level communication, interpersonal and negotiation skills
- Ability to work effectively and cooperatively in a team
- Ability to assist clients to formulate a care plan within their budget and manage client expectations empathically
- Excellent Computer literacy skills including Microsoft Office
- Commitment to embrace, model and work within the core values of McLean Care
- Integrity, Respect, Reliability, Courage, Compassion, Resilience
- Knowledge and understanding of Consumer Directed Care packages
- An awareness of Work Health and Safety, Duty of Care and Client and Staff rights
- Demonstrated experience in assessment, referral, and creation/coordination of human services
- Knowledge and experience in supervision of direct care staff providing services in people's homes
- An unrestricted driver's license
