Overview
Acute to Aged Related Services - Clinical Nurse Specialist 2 role at Hunter New England Local Health District. Location: Armidale Hospital. Employment Type: Permanent Part Time. Hours per week: 30. Remuneration: $57.11 - $58.99 Per Hour + Super + Salary Packaging.
Responsibilities
Provide advanced clinical care, leadership, and expertise in managing complex cases within the Acute to Aged Related Services (AARCS) team.
Deliver the first face-to-face assessment within one working day of referral and work with families, carers, and service providers to develop safe discharge plans.
Balance competing priorities, manage patients exhibiting challenging behaviours, and advocate for older patients while guiding and supporting colleagues.
Apply advanced assessment and case management skills to contribute to improving outcomes for older people.
Responsibilities (continued)
Participate in interdisciplinary collaboration to ensure patient-centred care and timely discharges.
Qualifications
Current registration as a Registered Nurse with AHPRA.
Relevant post-registration qualifications and experience appropriate to the role of CNS2.
