**Support Workers (Mental Health & High Physical Needs Experienced)**

**About the Role**

We are seeking multiple **male and female** **Support Workers** with **experience in mental health & high physical care** to support our participants in Wellard, Armadale, Cannington and Forrestfield. In order to regulate daily routines smoothly, **participants require intensive support** both at home and in the community. As a **Support Worker**, you will provide both emotional and practical support to participants with a mental health background. Your role is vital in promoting recovery-based practices, enhancing independence, and maintaining a safe, respectful, and empowering environment. You will assist participants with daily living activities, encourage community engagement, and support their overall social and emotional well-being.

**Support days**: 7 days a week (Mon-Sun)

**Support times**: Morning, Afternoon, Evening, Night (Sleepover)

**Your responsibilities will include**:
**Mental Health Support**:

- Building trusting relationships and providing emotional support to participants with mental health challenges.
- Supporting recovery-based practices, encouraging goal setting, and promoting self-advocacy and independence.
- Assisting participants in managing their daily routines, stressors, and community engagement activities.
- Monitoring mental health status and responding appropriately to signs of distress or crisis, following escalation procedures as required.

**Complex Physical Care**:

- Providing high-standard personal care including showering, dressing, grooming, toileting, and feeding (including PEG feeding, if trained).
- Supporting mobility needs using safe manual handling practices and assistive devices such as hoists and transfer aids.
- Administering or assisting with medications in line with authorised care plans and organisational policies.
- Monitoring and supporting participants with chronic health conditions (e.g., diabetes, epilepsy, pressure care).

**Daily Living and Community Participation**:

- Assisting with meal preparation, household tasks, and maintaining a clean and safe living environment.
- Supporting participants with transport and participation in community, social, and recreational activities.
- Encouraging positive routines, life skills development, and social inclusion.
- **Communication and Documentation**:

- Maintaining accurate and timely records including progress notes, incident reports, and health observations.
- Reporting changes in participants' health, behaviour, or support needs to the appropriate team or supervisor.
- Collaborating with family members, allied health professionals, and team members to deliver coordinated care.

**Compliance and Professionalism**:

- Following individual support plans, risk assessments, and safety protocols at all times.
- Upholding NDIS Practice Standards, participant rights, privacy, and dignity.
- Attending required training, supervision, and team meetings to ensure continuous professional development.

**Essential Requirements**:

- Certificate III or IV in Mental Health, Individual Support (Disability or Aged Care), or equivalent (or currently working towards)
- Current First Aid and CPR certification
- NDIS Worker Screening Check and Working with Children Check (if applicable)
- Valid Driver's Licence and access to a reliable, insured vehicle
- Manual Handling and Medication Administration Training (or willingness to obtain)
- Strong understanding of the NDIS framework and person-centred care principles
- Excellent communication skills and the ability to build rapport with participants from diverse backgrounds
- Availability to work flexible hours, including weekends, evenings, and public holidays as required
- Commitment to participant rights, dignity, and independence

**Work Environment**:

- Community-based, in-home, or supported accommodation settings
- Flexible shifts including weekends, evenings, and public holidays as required
- May involve transport support, personal care, or medication assistance (depending on individual needs)
