To meet our growing number of home care package clients, Alpha Omega Management Consulting is looking for an aged care team leader cum a registered nurse in a part time capacity in Melbourne.
**Position Purpose**:
The Aged Care team leader cum a registered nurse is responsible for supervision, support and mentoring of staff within the Melbourne aged care team, decision making, in liaison with other team leaders as appropriate, in area of responsibility. The position holder is responsible for the team's provision of high-quality client services through assessment, referral, co-ordination, monitoring and case management (including package financial management). This may be achieved through visiting clients at home, developing a care plan and implementing service delivery.
**Nursing responsibilities**:
- Deliver holistic quality nursing care in accordance with the Australian Nursing and Midwifery Council (ANMC), National Competency Standards competencies, NSW Health policies and procedures, to achieve patient/client health outcomes.
- Communicate effectively with patients/clients to provide an understanding of service delivery processes and clinical interventions, to ensure patient needs are identified and requirements are met.
- Consult and liaise with health care professionals within the HCP and/or NDIS team to ensure continuity of care to patients/clients.
- Maintain clear and concise patient records that meet legislative requirements to ensure continuity of safe patient care.
- Utilise a systematic approach to assess, analyse and propose alternative solutions to situations that arise, in order to resolve patient care issues, within the scope of professional practice and under the supervision of the Registered Nurse.
**In-home aged care coordination**:
- End to end client services management: intake, service (and risk) assessment, reviews, service transfer and coordination of direct support activities (working with Rostering Officer) and excellent understanding customer's monthly statement.
- Participate in company after hours programs.
- Working with customers, their supports and other providers (e.g. LAC, ACAT), contractors and subcontractors to achieve their goals in line with their individual plans.
- Maintain accurate and up to date case notes on client progress on the CRM system.
- Maintain an up-to-date knowledge of local services and pathways that customers can access and current sector systems and practices.
- Facilitate care worker support and supervision including accurate documentation, reviewing their progress notes and record keeping.
- Maintain internal filing, including accurate client documentation and other miscellaneous filing.
- Complying with all relevant legislation.
- Support other team members and the team leaders when required.
To be successful for this role, ideally you have.
- Analytical thinking and problem solving skills.
- The ability to communicate clearly and concisely and to adapt** **communication skills to each client you work with.
- At least three years of work experience in the aged care sector in a similar role.
- Ability to speak a second language fluently.
In addition to the above, you must have the following pre-requisites.
- Current police check or willing to obtain one prior to the job offer.
- Receiving at least 2 doses of Covid-19 vaccination certificate unless an approved medical contraindication form is provided.
- Full driver licence.
- APHRA registration for nursing or working toward it.
