Cornerstone Community Services is an NDIS Approved Provider, and is available 24/7 to offer people living with disabilities the support they need to live their day to day lives.
- Through our person-centered program, we assist our participants with daily living activities including but not limited to cooking, cleaning, mobility, access to the community, honoring appointments and so much more.
- Satisfying the needs of the individuals we support is our first priority. We do this by ensuring participants' choices and independence are promoted in an atmosphere of dignity and respect as well as health and safety.

Cornerstone is committed to deliver a highly personal, responsive, and client-focused service to people needing to access disability support services. This role requires full time hours 40hrs a week, more hours available if needed.
**About You**:
We are looking for someone with leadership characteristics and is comfortable using Microsoft apps. Someone who has experience in successfully leading and managing a team of values-aligned staff. Highly developed written and verbal communication skills including the ability to develop and maintain positive relationships with clients and their representatives. We are looking for someone who is kind, polite, courteous, and eager to learn. Someone who genuinely cares about their clients is eager to listen and assist them in realising their full potential. A genuine desire to offer the highest quality assistance for persons with disabilities, as well as the capacity to create good connections with clients and other stakeholders, is required.
**Responsibilities**:
- Coach, mentor and develop staff, including overseeing new employee on-boarding and providing career development planning and opportunities.
- Ensure a reflective practice framework is evident through regular staff supervision at all levels within services and aligns individual efforts with organisational goals.
- Ensure staff training and development is prioritised in line with program requirements, individual aspirations and resources available.
- Ensure Cornerstone's expectations and requirements are clearly communicated to staff at all levels and reviewed through supervision, team meetings and appraisal systems.
- Provide oversight and direction to the employees in accordance with Cornerstone policies and procedures.
- Provide individual support services and direct care for people with disability
- Support the client in community participation programs
- Provide accurate and detailed incident reports, progress notes and shift handovers
- Supporting with Domestic activities and assistance with personal care
- Medication assistance
- Assist with meal preparation
- Willingness to work with individuals with complex behaviours of concern
- Maintaining records of client progress
- Social and Community participation and access - Must have a reliable vehicle
**Additional Responsibilities Include**:
- Build rapport and collaborate effectively with staff/management, participants, their family, and friends to achieve a positive outcome and ensure a smooth service delivery flow throughout the participants' plan.
- Assist with liaising with participants' families, friends or professionals to explore new goals, ambitions, promote and empower participants to achieve their goals and provide good communication.
- Increase participants skills with your active support and promote independence by enhancing their skills and personal development.
- Provide quality and professional support in line with participants plans, needs, and wants.
- Assisting participants with accessing their social community to engage with their friends and social groups to promote a healthier lifestyle.
- Implement strategies and programs to develop/enhance participant skills, identify areas of increased support and opportunities for services to help achieve their goals.
**Requirements**:
- Sound working knowledge and understanding of the National Disability Insurance Scheme (NDIS) and experience utilising a range of community-based supports to achieve client goals.
- Experience in successfully leading and managing a team of values-aligned staff.
- Demonstrated ability to work effectively in a busy environment and complete administrative tasks within designated timeframes with accuracy and high attention to detail.
- Highly developed written and verbal communication skills including the ability to develop and maintain positive relationships with clients and their representatives.
- Ability to work independently and effectively manage time
- The ability to support participants to achieve social and self-care goals
- Effective verbal and written communication skills
- Certificate III in Individual Supports /Disability Cert IV or equivalent
- Current First Aid and CPR
