**About the Role**

The Coordinator HUBs and Community is responsible to lead and develop teams that provide person centred active support across Help Disability Care's Centre Based Activities (HUBS) and Community Support Services. They are responsible for the management of all activities, staffing and resourcing in their site/s. They lead a diverse and inclusive team of people who support participants including those under the National Disability Insurance Scheme (NDIS). This role holds on-call requirements under a rotating roster for out-side business hours.

The Coordinator HUBs and Community is responsible for, but not limited to the:

- Coordinate business activities including, daily programs and attendance, rostering, health and safety, site maintenance and overall performance for your site/s
- Be responsible for the development of active support programs, community activities and participant NDIS goal setting
- Support Help Enterprises mission and values and uphold policies and procedures
- Ensure all sites meet work health and safety requirements, in collaboration with the Safety, Health, Environment and Quality team
- Implement growth strategies; identify areas of improvement
- Contribute to the financial performance and sustainability of each site/program, in collaboration with the General Manager - HDC and the Finance team
- Attend relevant industry network meetings and training, and develop and maintain collaborative partnerships with key stakeholders
- Support staff training and development, mentoring, behaviour and performance within your team
- Ensure the effective record keeping and data exchange, including minutes of all staff meetings and risk assessments of all activities
- Ensure timely and appropriate handling of incidents, feedback and recording as required in incident management systems and PRODA
- Ensure that participants are regularly communicated with in regards to program development, participant progress and organisational activities
- Oversee participants service plans and goals

**About You**

As a Coordinator HUBs and Community, you will be dedicated to ensuring equal access and opportunities for individuals with disabilities. Working collaboratively to create an environment where everyone can thrive and reach their full potential, regardless of any challenges they may face.
- Experience in managing staff and daily operations of disability or community support programs
- Demonstrated knowledge and understanding of the NDIS including.
- Excellent knowledge of IGP's
- Good knowledge of PBSP and Restrictive practice
- Undertaking of incident management and PRODA reporting
- Excellent demonstrated knowledge of implementing goal orientated activities and progress reporting.
- Excellent demonstrated knowledge of the Disability Quality and Safeguard framework
- High level written and verbal communication: the ability to communicate clearly and concisely, varying communication style depending upon the audience and to build and maintain relationships with other service providers and community organisations.
- Attention to detail
- High support needs skills are strongly desired, such as manual handling, medication assistance, enteral feeding, and adaptive communication.
