Are you a compassionate and skilled Clinical Nurse with a profound dedication to improving the lives of the elderly? SA Health invites you to join our esteemed Geriatric Outreach Service in the vibrant city of Adelaide, South Australia.
About the role:
Reporting to the Advanced Nurse Unit Manager, Complex and Restorative Care Service (CARE Service), you will play a pivotal role in delivering exceptional care to our senior members of the community. In the role you will collaborate with a multidisciplinary team of healthcare professionals, leveraging your expertise to enhance the well-being of our geriatric patients and their families.
In this rewarding role, you will have the opportunity to make a lasting impact by providing comprehensive assessments, developing personalised care plans, and fostering a supportive environment that empowers our elderly patients to thrive in their Residential Aged Care Home. Your commitment to patient-centred care, coupled with your clinical acumen, will contribute to the overall success of our healthcare initiatives.
About you:
- Registered or eligible for registration with at least 3 years' experience as a Registered Nurse with Australian Health Practitioner Regulation Authority and who holds, or who is eligible to hold, a current practicing certificate.
- Effective communication, problem solving, conflict resolution and negotiation skills.
- Ability to work effectively within a multidisciplinary team.
- Ability to prioritise workload and meet set timelines, whilst working under minimal supervision.
