About the Role
You will be based at Royal Prince Alfred Hospital (RPAH). It is one of the nation's most respected hospitals, with a distinguished history of serving the health needs of local, statewide, national and international communities. Our facilities and services are world class and our staff are dedicated, innovative and caring.
This position provides support to the palliative care service three days per week (two days inpatient and one day in the community). The remaining day provides support to the Renal Supportive Care (RSC) service. The RSC Social Worker is an ambulatory role, one day per week, and forms part of the Leading Better Value Care (LBVC) Renal Supportive Care initiative of NSW Health. The social worker supports patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD), as well as their families and carers, addressing psychosocial, emotional, and practical needs. Key responsibilities include comprehensive psychosocial assessments, counselling around treatment decisions, managing family and carer distress, and assisting with advance care planning. The social worker advocates for access to financial, housing, and community resources and provides bereavement support. They facilitate support groups for patients and carers, promoting peer connection and resilience, and contribute to education and research, supporting staff training and service development. While RSC shares principles with palliative care, it is distinct in focusing on quality of life and symptom management throughout the illness trajectory, not solely at end of life.

Ideal Candidate
- Bachelor or post graduate degree in Social Work that provides eligibility for full membership of the Australian Association of Social Workers (AASW) with experience at Level 2 or higher.
- Holds a current NSW driver licence (P1 and P2 acceptable).
- High level verbal, written communication, and interpersonal skills, including the ability to adapt communication style to suit different audiences and work effectively with a multidisciplinary team.
- Ability to initiate, lead, complete and evaluate quality improvement initiatives and/or research focused on patient/client outcomes.
