Supreme Care is currently seeking experienced and compassionate Support Workers to join our team in delivering high-quality home and community care services. We are particularly looking for candidates with experience supporting NDIS participants with mental health needs or providing in-home aged care.

Key Responsibilities

  • Provide in-home support to clients with mental health challenges or aged care needs, promoting independence and wellbeing.
  • Assist with personal care, such as bathing, grooming, dressing, and medication management.
  • Monitor client wellbeing and report concerns to healthcare professionals.
  • Develop and follow individualised care plans in collaboration with clients, families, and health teams.
  • Offer emotional support and encourage self-esteem, empowerment, and self-advocacy.
  • Communicate with family members, healthcare providers, and other stakeholders to coordinate care.
  • Accurately maintain documentation such as progress notes, care plans, and incident reports.

Essential Criteria

  • Minimum 1 year experience as a Support Worker (mental health or aged care).
  • Excellent communication and interpersonal skills.
  • Ability to work independently and within a multidisciplinary team.
  • Compassionate, patient, and professional approach.
  • Sound knowledge of mental health conditions and support strategies.
  • Strong problem-solving skills and adaptability.
  • Current Australian driver's licence and access to a reliable vehicle.

Desirable Qualifications

  • Certificate III or IV in Individual Support, Mental Health, Disability, or related field (or currently studying).
  • Prior experience working with NDIS participants.
