About the role
Empower Healthcare provides an innovative service model that focuses on improving people's health and wellbeing and increasing their quality of life. Working in a supportive multidisciplinary team you will be responsible for the delivery of Empower Healthcare's speech pathology services to home care clients in South Perth. Please note that this position will require you to work 7 hours in Residential aged care on some weeks.
Your responsibilities will include:
- Deliver high quality speech pathology services and programs including assessment and on-going management of dysphagia, language, and speech.
- Work with individuals aged 65 years and over in a person-centered approach.
- Work autonomously to prioritise and complete tasks, maintain records, and provide patient reports.
- Work collaboratively with our clinicians to develop innovative and progressive care plans that aim to promote the independence and wellbeing of patients.
You will be provided with support from our home care team who will schedule your appointments and plan an efficient route.
About you
We are looking for someone that is passionate about making a difference in the lives of our people. You will have an enthusiastic and proactive attitude, be outcome focused and dedicated to delivering an exceptional service. You must have:
- A current Driver's license and own vehicle
- A speech pathology degree accredited by Speech Pathology Australia
- Certified Practicing Speech Pathologist status with Speech Pathology Australia
- A recent National Police Certificate within the last 3 years
