As a Disability Support Worker at Proactive Community Care Pty Ltd, you will play a vital role in providing essential support and care to individuals with disabilities. Your primary responsibility will be to assist participants in achieving their goals, promoting their independence, and enhancing their overall quality of life. Position Overview:

We are seeking a compassionate and dedicated Disability Support Worker to join our team in providing essential care and assistance to individuals with disabilities. The role involves supporting clients in their day-to-day activities, promoting independence, and enhancing their quality of life. The ideal candidate will possess empathy, patience, and a genuine commitment to making a positive difference in the lives of those we serve.

If you have a strong passion for positively impacting the community, your daily responsibilities could encompass a variety of tasks. These may involve facilitating access to community amenities, providing personal care, assisting clients with grooming, arranging social outings with friends, accompanying them to medical appointments, aiding in household tasks, administering medications, and facilitating transportation to enhance their overall quality of life.

Key Responsibilities:

Personal Care Assistance: Assist clients with personal hygiene tasks such as bathing, grooming, toileting, and dressing, while ensuring dignity and respect at all times.

Mobility Support: Provide assistance with mobility aids, transfers, and positioning to help clients move safely around their environment.

Medication Management: Support clients with medication administration as per their care plans, ensuring compliance and adherence to prescribed regimens.

Emotional Support: Offer companionship, active listening, and emotional support to clients, fostering a trusting and supportive relationship. Foster a supportive and empowering environment that respects the dignity, rights, and choices of participants.

Daily Living Activities: Assist with household tasks including meal preparation, cooking, cleaning, laundry, and grocery shopping, adapting support to meet individual needs and preferences.

Community Engagement: Facilitate participation in social and recreational activities, outings, and community events to promote social inclusion and integration.

Documentation and Reporting: Maintain accurate records of client interactions, progress notes, and any incidents or concerns, ensuring compliance with organizational policies and procedures.

Collaboration: Work collaboratively with multidisciplinary teams including healthcare professionals, family members, and external service providers to coordinate holistic care and support plans.

Safety and Risk Management: Identify and mitigate potential risks to client safety and well-being, implementing appropriate strategies and interventions as required.Monitor and report any changes in participants' health, behavior, or needs to the appropriate supervisor or healthcare professional.

Professional Development: Participate in ongoing training and professional development opportunities to enhance skills and knowledge in disability support practices staying informed about best practices and emerging trends.

Adhere to organizational policies, procedures, and standards of conduct at all times, promoting a safe and inclusive work environment.

Qualifications and Requirements:

  • Previous experience working in the disability sector or a related field is preferred.
  • Certificate III or IV in Disability Support, Individual Support, or a relevant qualification is highly desirable.
  • Demonstrated experience working in disability support or a similar caregiving role, preferably in a community or residential setting.
  • Strong communication and interpersonal skills, with the ability to build rapport and establish trusting relationships with participants and their families.
  • Empathy, patience, and a non-judgmental approach to supporting individuals with diverse needs and backgrounds.
  • Compassionate and empathetic attitude towards individuals with disabilities, with a commitment to promoting their autonomy and self-determination.
  • Ability to adapt to changing circumstances and respond effectively to challenging situations with resilience and professionalism.
  • Basic understanding of person-centred approaches, trauma-informed care, and relevant legislation (e.g NDIS Quality and Safeguarding Framework).
  • Commitment to upholding the rights, autonomy, and dignity of clients, while promoting a person-centred approach to care.
  • Current First Aid and CPR certifications, or willingness to obtain upon commencement.
  • Valid driver's licence and access to a reliable vehicle for transportation purposes (if required).
  • Blue Card/Yellow Card (Must have or willing to obtain)
  • Evidence of your Right to Work in Australia (Must have)
  • Successful completion of relevant background checks and screening processes as per regulatory requirements.

Working Conditions:

  • This position may involve shift work, including evenings, weekends, and public holidays, to accommodate the needs of clients.
  • Work environments may vary and could include private residences, group homes, day centers, or community settings.
